Become a Resident
We want you, our new residents to have a comfortable moving-in process. Please use this checklist as a tool and a guide.
Step 1 – Resident Application – Print and fill out the resident application and mail or drop it off at our office along with your $200 non-refundable check.
Step 2 – Print out and have your physician complete our Doctor’s Evaluation. Mail in or drop off completed form to our office.
Step 3 – Drop off or mail in your Security Deposit of one month’s rent.
Step 4 – Print and fill out our Vial of Life form, then mail or drop it off at our office.
Step 5 – Provide us with your Power of Attorney, Attorney or Guardianship, if applicable.
Step 6 – Provide us with advance directives (Living Will, DNR, DPOA/HC), if applicable.
Move-In Day!
- On the day of move-in, you will need a check for your first month’s rent unless you have already taken care of it. We will pro-rate your rent.
- You will need to sign our Residency Agreement. We do not have yearly leases.
- Make sure you have placed the electric in your name if you live in Independent Living. And contact the phone company for your own account.
- Please bring your address book to notify all your family, friends, clubs and church of your new address. Address change cards are available at the business office in the Village.
